Note 5 wireless charging pad issues (Vote here to help us narrow down source)

I have posted in this thread a question about the bundled Note 5 wireless charger. It seems as some people are having issues with the pad.

Please vote to help us narrow down how or when issues might arise. Issues such as stops charging, blinking blue light, doesn't charge or I wake up to find my phone not fully charged
